I ve just bought a new mouse because my old one really sucks.
the new one has a lot of Mhz of polling rate and a lot of dpi
But i can t use it with my old config, because is worst than the old one.
So, with some trys, i found that if i change cl_maxfps from 77 to 154, mouse works correctly and it s really smooth.

now i use
vid_frequency 75 hz on a 22'' tft
in_mouse 3
cl_ max_fps 154 (my hardware is not really new: ATHLON 2200+ and geforce MX..quite old)

no -dinput , -m_smooth , m_filter, m_rate or any other settings, just mouse settings (rate 1000 and 3200 dpi)

is it correct?
there is something else i have to do?

thank you !

I have found that it is most important to have FPS be a multiple the hertz of the monitor. Mouse hertz comes second. Thus I have 1024x768@100Hz cl_maxfps 100 and 500hz mouse. Super smooth.

you could try cl_maxfps 150, witch is maybe more smooth if u have 75hz refresh.
And try lowering m_pitch and yaw instead of sensitivity going to a high dpi mouse
