Being a QW player on a new Operating system can sometimes be annoying
Here is what problems I've come across so far and have i solved some

*Mouse polling rate
managed to set 1000hz by following the steps in this thread:
http://www.ngohq.com/news/12163-how-force-1000hz-usb-sample-rate-windows-vista.html
and http://forum.overclock3d.net/showthread.php?p=167571
(Applying that fix with the listed updates installed caused all USB-devices to stop working in both failsafe and normal windows. Which is tricky to rollback if you have no ps2 keyboard available)

*Mouse Accel
With Microsoft mouse and no driver installed, i got the tip to uncheck the 'improved mouse precision' box in mouse settings. Things got better, but there definitely was some acceleration left.
With logitech mouse and Logitech set point drivers and acceleration set to 'none' in settings things are a bit better, but still not as good as in XP (with accel fix)

update: using -noforcemparms -noforcemaccel -noforcemspeed in the cmdline fixed the problem. Atleast for the logitech mouse.

*Open GL driver
After installation of vista, I had to download latest drivers from www.nvidia.com to make it run.

*Microphone (ventrilo)
Windows Sound recorder works but with Ventrilo, voice activation would either enable send all the time, or not at all. Propably just me being lost in the settings.

*Performance
The game runs very smooth and I have not experienced any FPS drops.

*Mouse polling rate
This was no problem, just using USB Mouse Rate Adjuster Setup (originally for XP?)

*Mouse Accel
Dunno what it is, but the feeling is somewhat different. I can't tell if it's accel or whattafack. But it feels enuff good for me to play with.

*Open GL driver
Had no problems with this, except when tabbing in/out of QW. One time it may render bottom half of the screen black, the other time some "Vista gadgets" keeps on wanting to be visible in QW, and the third time it was like holding a large magnifying glass infront of the screen; the image was pinched.

*Performance
The games runs just as smooth as on XP. But once every hour (or just sometimes) I get like 10 fps, requiring a tab out, tab in to fix. Strange. And it has occurred more than once that the comp just die. Poff! Funny during eql, ye!

Probably the most annoying thing for me is that whenever I quit QW in Vista, the desktop gamma isn't restored (so it's really bright, using my QW gamma). I then have to go into nvidia display properties and click the brightness slider to restore it to normal.

I also get occasional little fps drops or stutters. HOWEVER this is likely due to also now using unlimited fps, maybe because of dual core cpu too.

One thing you could try is manually editing the driver inf file to force 144hz at your chosen resolution.
To do this:

-Extract nvidia driver to temp directory
-Edit nv_disp.inf
-Find the line that says nv_modes
-Change the relevant resolution to be equal to 80xx where xx is the hex value for your desired refresh rate (90 for 144hz). So lets say you want 800x600 @ 144hz, then you need "800x600=8090". Like this:

HKR,, NV_Modes, %REG_MULTI_SZ%, "{*}S 640x480 1280x768 1280x800 1360x768 1920x1200=1;800x600=8090;848x480 960x600 1024x768 1152x864 1280x960 1280x1024 1600x1200 1920x1440 2048x1536=3B;{191-19E,400-42F}S 640x480x64 1280x768x64 1280x800x64 1360x768x64 1680x1050 1920x1200x64=1;800x600x64 848x480x64 960x600x64 1024x768x64 1152x864x64 1280x960x64 1280x1024x64 1600x1200x64 1920x1440x64 2048x1536x64=3B;"

IMPORTANT: Note how I seperated the resolution we are modifying from the rest using a semicolon, otherwise multiple resolutions might get changed.

Now just install the driver using the setup.exe in your extracted package. Hopefully this will give you 144hz (but don't count on it, I've no idea if Vista has some other way of limiting to 100hz).
